Increased Health Equity Across Diverse Populations

The Challenge

Resolving Inequity Drivers

Multiple hurdles impact a patient’s motivation and ability to seek care.
At least 80 percent* of these factors are non-clinical, including:
  • Social determinants of health, which disproportionality impact rural and minority communities
  • Lack of convenience, which reduces motivation and the likelihood to seek care
  • Fear, which can be difficult to overcome without consistent and persistent reassurance
  • The inability to deliver just-in-time communications that ensure education and clarity of direction
  • Lack of awareness, which limits a patient’s understanding of the size and scope of the options available to them

But providers have limited time, visibility, and connection with patients. The average length of a primary care visit is 18 minutes; the average length of a hospital stay is 5.5 days.

It is impossible for a provider or supporting clinical staff to create and maintain the ongoing connection needed to resolve the non-clinical barriers contributing to 80 percent of a person’s health risk.

The Guideway Solution

Guideway Care’s Solution to Scaling Health Equity

Through our work across multiple healthcare organizations, communities, and target conditions, we learned that there is no singular channel that can reduce inequity while improving a person’s motivation to manage their health actively.

The complexity of inequity drivers requires a balanced, scalable approach that combines trained care guides, technology, and clinical and non-clinical protocols. This mix of levers can be tuned depending on the risk, complexity, and non-clinical barriers presented at the individual level. The flexibility inherent to this approach enables performance at the lowest total overall cost, and it creates the continuous connection required to address—holistically—inequity in access, resources, and understanding.

Illuminating the Patient Journey

For every patient, we collect:

ISSUES — patient concerns that they believe will be resolved without care guide intervention. Care guides track issues to ensure patient resolution; they are equally important as barriers to maintaining the patient stability.

BARRIERS — patient concerns requiring care guide action to resolve. While physical symptoms represent the majority of issues, they represent a minority of barriers.
Physical symptoms, as a percent of all barriers, represent a small fraction. And when identified, these barriers are escalated appropriately. Eighty percent of the work typically done by nurses and other health care professionals is completed by care guides.

The Results

Value Delivered

Guideway Care’s proven, comprehensive, person-powered, peer-to-peer, AI system-supported approach to identify issues and resolve underlying barriers (non-clinical health inequity and clinical drivers) results in improved patient outcomes and satisfaction, at a fractional cost of traditional models. We contractually guarantee significant ROI in the following models:

MCOs/Value-Based Care
Decrease total cost of care by proactively navigating and activating members to eliminate barriers, which results in care-gap closure, reduced acute care utilization, and increased adherence to clinical protocols.
Hospital Readmissions
Eliminate avoidable readmissions to improve quality and decrease cost under both the Hospital Readmissions Reduction Program (HRRP) and Medicare Advantage plans.
ER Utilization
Redirect unnecessary and avoidable ER utilization by proactively and effectively directing patients to the appropriate site of care to maximize patient satisfaction, reduce cost, ensure quality outcomes.
Patient Satisfaction
Deliver consistently higher patient satisfaction and retention through persistent peer-to-peer interactions at a lower cost, higher reach rate.
Cancer
Improve performance under value-based oncology models, reduce patient migration, reduce missed appointments, and increase patient satisfaction.
Bundles
Optimize performance in bundled payment models under value-based care to deliver shared savings.

Outcome Highlights

Through continuous, persistent, and effective connection delivered at scale, Guideway solves for the variation in risk that is out of your control. As a result, we:
  • Reduce health inequity — find and solve the countless practical issues and barriers embedded in inequity drivers
  • Close gaps — establish a peer-to-patient relationship that more efficiently closes known gaps and uncovers hidden risks
  • Drive down high-cost chronic care — proactively escalate risk and route to appropriate clinical resources

  • Increase adherence and compliance — ensure follow up and understanding
  • Improve satisfaction — establish stronger connections and relationships
  • Increase nurse capacity — by serving as a 5x force multiplier for clinical capacity
