Guideway Care’s cancer care guidance program has its roots in the Patient Care Connect (PCC) Program, a CMMI-funded pilot that spanned twelve cancer centers across five states. Independently audited and validated savings reached nearly $4,100/quarter per patient during the acute care phase, with similar savings in the last six months of life (more than $8,000 per patient), as well as HCAHPS care transitions scores in the 95th percentile or above among U.S. hospitals. The program includes barrier resolution, patient education, advanced care planning, connection with psychosocial support resources, and other interventions needed to support alternative payment models such as the Enhancing Oncology Model.
Guideway’s care guidance program for CABG patients includes practical barrier resolution, patient education (including wound care, infections, etc.), patient-reported wound monitoring, medication adherence, dietary instruction, and other non-clinical interventions for common comorbidities, including diabetes and COPD.
