Acute Myocardial Infarction (AMI)
Guideway’s care guidance program for AMI patients includes practical barrier resolution, patient education (general education of heart failure, ischemic heart disease, stroke, etc.), smoking cessation, medication adherence, and other non-clinical interventions for common comorbidities, including COPD, diabetes and anemia.
 
Cancer

Guideway Care’s cancer care guidance program has its roots in the Patient Care Connect (PCC) Program, a CMMI-funded pilot that spanned twelve cancer centers across five states. Independently audited and validated savings reached nearly $4,100/quarter per patient during the acute care phase, with similar savings in the last six months of life (more than $8,000 per patient), as well as HCAHPS care transitions scores in the 95th percentile or above among U.S. hospitals. The program includes barrier resolution, patient education, advanced care planning, connection with psychosocial support resources, and other interventions needed to support alternative payment models such as the Enhancing Oncology Model.

Coronary Artery Bypass Grafting (CABG)

Guideway’s care guidance program for CABG patients includes practical barrier resolution, patient education (including wound care, infections, etc.), patient-reported wound monitoring, medication adherence, dietary instruction, and other non-clinical interventions for common comorbidities, including diabetes and COPD.

COPD

Guideway’s care guidance program for COPD patients includes barrier resolution, extensive patient education on a variety of topics, pulmonary rehab accountability, smoking cessation, medication adherence, and other non-clinical interventions for several common comorbidities, including cardiovascular disease, depression/anxiety, diabetes and osteoporosis.

Guideway’s care guidance program for heart failure patients includes practical barrier resolution, patient education, medication adherence, smoking cessation, dietary instruction, and other non-clinical interventions for several common comorbidities, including COPD, diabetes, high blood pressure and obesity.

Obstetrics

Guideway’s care guidance program for obstetric patients is a wellness program designed to enhance the care experience for patients and their families, increase engagement, and ensure the mother-to-be feels supported, informed and prepared for every step in her journey. The proactive non-clinical interventions included in the program are designed to support expectant mothers and their families with improved communication, education and personal assistance. Our program invites expectant mothers to see your clinical resources as key tools to help her through every aspect of pregnancy, and even into the first transitions with her newborn. The program is comprised of person-to-person and technology-enabled interactions, ensuring the patient feels heard, valued and prepared through every step of their care journey.

Pneumonia

Guideway’s care guidance program for pneumonia patients includes practical barrier resolution, medication adherence, smoking cessation (if applicable), and patient education (fluid intake, heart failure symptoms, COPD, bronchioectasis, etc.).

THA/TKA

Guideway’s care guidance program for THA/TKA patients includes practical barrier resolution, patient education (including wound care, infections, etc.), patient-reported wound monitoring, medication adherence, and other non-clinical interventions for common comorbidities, including hypertension, diabetes, obesity, deficiency anemias, hypothyroidism, and depression.

After extensive training in motivational interviewing and Respecting Choices, Care Guides resolve patient / member barriers related to Social Determinants of Health (SDOH) and disparities, overcome clinical and non-clinical barriers to quality care and promptly address the physical, practical, emotional, informational, spiritual and familial issues that impact patient outcomes.

Proprietary Patient Activation Platform

Guideway’s proprietary platform automates protocols to uncover both non-clinical and clinical issues and barriers for Care Guides. The platform automates the ingestion of tasks, the next step process, clinical escalation, and barrier resolution. The automated prompting and workflow design allows the Care Guides to build peer-to-patient relationships, while stacking non-clinical and clinical assessments in structured reportable data.

We are able to resolve non-clinical patient barriers and automate the escalation of clinical patient barriers to our partner clinical teams.
