Our Care Guides were able to achieve significant results in Cardiac Care Bundles. Through a 13 week protocol driven approach we were able to significantly drive down readmissions by building peer-to-patient relationships.
Within the Obstructive Lung Disease bundle, our Care guides were able to decrease readmissions by 36%. Addressing non-clinical barriers to care and escalating clinical barriers makes a difference.
In a 3 year published study our Care Guides delivered amazing patient satisfaction and reduced hospitalizations in oncology by 55%.
