This system wanted to improve patient satisfaction scores across measures focused on discharge, the likelihood that a patient would recommend the provider, overall hospital rating, and the transition of care.
Care guides performed post-discharge outreach to ensure patient understanding of discharge instructions and follow up care. Additional insights into the patient experience were captured and reported back to the provider.
Within two months, Guideway Care increased patient satisfaction scores by an average of 10 points, which resulted in an average percentage improvement of more than 15%.
A patient’s daughter had the incorrect contact information for the provider to which her parent was referred. The daughter was frustrated.
