High-Touch, Tech-Enabled Care Guides Resolve Barriers to Accessing Health Care for Tennessee’s Most Vulnerable Population

Vulnerable communities in Tennessee experience health disparities due to the conditions where they live, learn, work and play, affecting a wide range of health risks and outcomes. Populations such as Native American, Black and African American, Hispanic and Latino, the disabled, low-income and less educated residents are not able to have access to basic healthcare.

Improving health equity for these vulnerable populations in Tennessee and other parts of the U.S. is crucial to strengthen the fabric of communities and grow the State’s economic position.
