Guideway’s Nurse Triage add‑on provides remote, highly trained registered nurses who collaborate with our Activation Care Guides. When our non‑clinical Activation Care Guides identify potential clinical red flags, such as persistent fever, worsening dyspnea or uncontrolled pain, our nurses evaluate symptoms, apply evidence‑based protocols and advise on the appropriate level of care. This seamless clinical escalation helps prevent deteriorations that would otherwise lead to readmission.
What We Do
- Review clinical escalations flagged by Activation Care Guides.
- Conduct telephonic assessments using standardized triage protocols to determine urgency.
- Provide immediate guidance, whether recommending same‑day follow‑up, ED visit or continued home care.
- Coordinate with physicians and care teams to adjust medications or treatment plans.
- Document findings and maintain communication loops with the Guideway team.
