BIRMINGHAM, Ala., May 17, 2021: After joining forces in 2019, Guideway Care and Sequence Health are now combining their solutions under the Conversion Healthcare brand umbrella to better communicate the depth and breadth of their end-to-end patient interaction capabilities.
Guideway Care has grown continuously since its inception as a care guidance solution developed as an outcome of a Centers for Medicare & Medicaid Services (CMS) Innovation Center (CMMI) grant. Leveraging its success with cancer care guidance, Guideway Care has expanded as a turn-key, technology enabled services solution for patient activation and care navigation across diseases, conditions, and avoidable health events.
